Could I have your valuable opinions as to the best way to translate "declaración" in this case.

My guess is this is just a form-
Even declaraciones juradas with strong legal effect (as tax returns) are just forms you have to sign [w/o other formalities like presence of a notary/fedatario], IMO affidavit sounds like too much for what a plumber has to produce.

I don't think a simple written statement would be enough. Probably they are requesting a "declaración jurada."

Hi, Counsel. I understand this concept the way Marian says. I have signed many "declaraciones juradas" - they are simple, tax return-like forms, in the sense that you are making a formal representation.

affidavit; represention

as the title of the document, I would use affidavit. In the body of the text, I would generally use representation.
